Office Hours


The office is open:

Monday: 9:00 AM to 4:00PM

Tuesday: 9:00 AM to 3:00PM

Wednesday: 9:00 AM to 4:00PM

Thursday: 9:00 AM to 4:00 PM

Friday: Closed

Saturday: Closed

Sunday: Closed


- Schedule your follow up appointment at the end of each clinic visit.

- Confirm office location with each appointment.



Payment

- Insurance co-pays and deductibles must be paid prior to the time of service.

- WE NO LONGER ACCEPT CHECKS.

- We accept Mastercard and Visa.

- If you have a current balance with our office, we ask that a payment is made at each visit.



Late appointment arrivals

- We will make every effort to see you when you arrive.

- Please be advised that it may be necessary to reschedule your appointment if you are more than 15 minutes late.



Missed appointments

- Please contact our office 24 hours in advance if you are canceling your appointment.

- Missed Appointments will result in a $35.00 cancellation fee.
