Payment:
Insurance co-pays and deductibles must be paid prior
to the time of service.
We no longer take checks more than $100.00 unless
they are certified checks.
A fee of $25.00 for returned checks is assessed.
We accept Mastercard and Visa.
Late appointment arrivals:
We will make every effort to see you when you arrive.
Please be advised that it may be necessary to
reschedule your appointment if you are more than 30
minutes late.
Walk-in visits:
We will make every effort to see you as soon as
possible.
Patients with scheduled appointments will be seen first.
If you are experiencing an emergency, please call the
office prior to making your emergency visit.
Missed appointment:
Please contact our office 24 hours in advance if you
are canceling your appointment.
Missed appointments will result in a $25.00
cancellation fee.
